How to set up your Patient Portal?

  1.  Advanced MD | Patient Portal – you will use your user name and password
  2. You will be sent a second authentication code via email (you chose during set up)
  3. Enter that into the second screen (shown below)- select verify
  4. At the top of your Patient Portal you will see a icon “Messages” select “Ask a Question” on the far right of screen.

What is Ascend Medical’s philosophy of care?

At Ascend Medical, you are at the center of everything we do. Our priority is your health and wellness. The goal of the doctors, nurses, and medical assistants on your care team is to help you live an optimally healthy, thriving life. This includes providing timely, comprehensive primary care when you’re sick and helping you stay well in the future through a healthy lifestyle. And if needed, we have a close relationship with a multispecialty network of physicians to partner with in your care.

How does Ascend Medical Provide treatment?

For each visit, we connect with you virtually via our state-of-the art telehealth. We assess your medical needs while gathering other medical information, answer all your questions and if we can treat you virtually we take care of everything you need during the visit including sending prescriptions to your local pharmacy.

 

If your locally-based Virtual Care Provider determines you need an in-person home visit, our Care Team will be scheduled at your convenience to come to your home, work or anywhere you need, and can provide care for you and your whole family.

How is Ascend Medical different from other practices?

Other practices have standard business hours, expect you to work around their schedule, have you take time off work to see them, drive to their office and wait in the lobby before receiving care. At Ascend Medical, we reimagined primary and urgent healthcare, pediatrics and mental health therapy and put you first. We have all your healthcare teams in one place because we believe healthcare should be easy to access and should encompass the body and mind for all ages. We also believe you should not have to wait to get an appointment, so we provide in-person care to you and your children right in your home, work or anywhere around town you need care or via our 24/7 virtual telehealth.

Will Ascend Medical come to my home or work to provide care?

Yes.  Our Mobile Care Team will meet you where you are – at home, in your office or anywhere else – to bring you the highest quality comprehensive healthcare, right when you need it. Plus, membership gives you 24/7 access to your care team with telehealth appointments.

Can I access Ascend Medical Care if I’m out of town?

Yes. You can access your Care Team 24/7 virtually via telehealth from anywhere in the world.  Plus, for your convenience you can have an in-person Mobile Care Team visit in any city with our mobile service – Click Locations for cities where Mobile Care Team service is offered.   

Does Ascend Medical offer Pediatrics?

Yes.  Ascend Medical offers Pediatrics (newborn to 17 years-old), so your children can relax in your home as your Pediatric Care Team provides in-person care for everything your little ones need.  That’s right, we come to your home.  Home visits provide you and your child timely, in-person access to all services that can be done in a physical office.  These well-child visits are for routine exams and minor issues including screenings and assessments, with a focus on prevention, growth tracking and development to identify any areas of concern.  

 

Plus, you get 24/7 virtual telehealth access to your Pediatric Care Team so you can connect with your Pediatric Care Team whenever you need them; like those middle of the night fevers, unexpected bumps and scratches, and more.

Does Ascend Medical offer Urgent Care?

Yes.  Our providers offer primary healthcare and can address a wide range of urgent care needs.  We also offer pediatrics and mental health therapy.  We have all your healthcare teams in one place because we believe healthcare should be easy to access and should encompass the body and mind for all ages.

What should I expect from my Care Team at Ascend Medical?

You should expect prompt, friendly care from a team whose is focused on you. The members of your care team have a calling to provide the best possible health care. During your visit, you will have time to share your concerns and ask questions. Your care team will have time to explore any issues to address your needs. After your visit, you will receive prompt follow-up as needed and your care team will welcome you back whenever you need us.

Do I need to select a primary care provider?

Although we don’t require members and non-members to select a primary care provider, we strongly encourage selection of an Ascend Medical provider you are comfortable with and who can collaborate with you and your family for many years to come on all your healthcare decisions.  We know your healthcare is important, so we have the best providers from your local area ready and happy to partner in your healthcare journey.

Is my virtual telehealth visit covered by insurance?

Most insurance companies pay for virtual visits. It is always best to confirm your benefits with your insurance company prior to care. After your visit, Ascend Medical will file a claim with your insurance provider. Any unpaid balance after the claim is paid will be the patient’s responsibility.

Do you take Medicare?

We do not currently accept Medicare, but are in-network with Aetna Medicare Advantage and Humana Medicare Advantage plans.

Does Ascend Medical offer Flu Shots?

Yes, we offer flu vaccine which is covered by most insurance plans. Alternatively, if you prefer not to bill insurance, the out-of-pocket cost is $45 for a regular vaccine and $75 for high-dose vaccine. Please check with your insurance provider if you have questions concerning your coverage.

Does Ascend Medical offer COVID Vaccines and Tests?

Yes.  We offer COVID-19 Testing and Vaccines.  

 

COVID-19 Vaccines we offer:

  • Pfizer 1st and 2nd Dose
  • Moderna 1st and 2nd Dose
  • Johnson & Johnson 1 Dose Vaccines
  • Group and Event On-Site Vaccination Clinics

 

COVID-19 Testing we offer:

  • Rapid Antigen Testing (Results in 15 minutes!)
  • PCR Testing (Same or Next Day Results)
  • Antibody Testing
  • Group and Event Testing

Why is there an Annual Membership fee?

The annual membership fee allows us to provide healthcare that revolves around you through state-of-the-art technology, in-person home visits and 24/7 virtual telehealth access.  Your membership helps develop digital health tools and also enhanced services like your Healthcare Navigators and 24/7 access that Ascend Medical offers to its members.

What if I can’t afford to pay the membership fee?

Everyone can get care at Ascend Medical.  While our members are happy to have the advantages of membership, having a membership is not required to receive care from Ascend Medical.  Ascend Medical is committed to serving our communities, regardless of financial means.  You can always self-pay if you are without insurance as well and get the care you need.  

What insurance plans do you accept?

Ascend Medical accepts health plans from most insurance providers.  Some of the major insurance plans that we accept can be found on our Insurance page, but because insurance plans are updated regularly, we encourage you to call us at 855-750-WELL so we can answers all your questions.

 

No insurance? No problem.  We offer self-pay options and a membership that will save you money so you get the healthcare you need, no matter what.

How long does it take to be billed?

The time it takes to receive a bill varies on your insurance company. Your insurance determines whether or not you are financially responsible for a balance of services based on your deductible and coverage.
