Ascend Medical is committed to providing the highest quality patient care and to acting with absolute integrity at all times. Your privacy is important to us. This Privacy Policy describes how we collect, use, disclose and protect your Personal Information. It applies to all Personal Information processed by us on any of our Services, as well as written, electronic, and oral communications. Collect Information and Use

Ascend Medical collects the domain name of a visitor's server whenever possible and maintains it in our website logs. Ascend Medical, directly or through our partners, analyzes our website logs to constantly improve the value of the information available on the website. If you access a link to a partner's website, that website may have additional privacy policy information. Ascend Medical's logs are not personally identifiable, and we make no attempt to link them with the individuals that actually brows the site. Ascend Medical collects very little personally identifiable information from visitors to its websites. Usually, such data is only collected when a visitor requests information, registers for specialized areas on the site, buys a membership, fills out a survey or form or submits a resume.

Information you provide to us when you register with us for a Ascend Medical account, when you use our Services, when you participate in surveys or promotional activities, or when you otherwise choose to submit your information to us.

  1. When you sign up for Ascend Medical account we may collect your name, address, phone number, title, birth date, gender, credit card information, together with other information such as occupation, industry, and personal interests.
  2. When you use our Services, such as logging into your account or speaking with a Ascend Medical membership advisor, we may collect your name, address, birth date, credit card information, etc., to verify your identity and provide Service to you.

Use of Information Collected

The domain name of the server from which a visitor comes is used to determine the number of visitors to our site, and the source of that traffic.

Email addresses provided by visitors are used to:

  1. Respond to comments submitted through feedback and survey forms, when appropriate
  2. Distribute personalized emails about services, promotions and health care information to those who request such personalized emails

Visitors who complete a survey or form, and indicate a willingness to participate in future surveys or receive future emails, are asked to provide their name, email, phone number, etc. That information may be shared with our marketing department and partners who conduct business on behalf of Ascend Medical.

Removing Your Name From Our Files

If you no longer wish to receive emails from Ascend Medical, please let us know by sending an email message to compliance@ascendmedical.com. Please be certain to provide the following, so we can honor your request:

  1. Your full name
  2. Your email address
  3. A sentence asking that your email address be removed from our files

Website Security

Areas of our website that collect personal information are encrypted. Encryption prevents your information from being intercepted. We store all of your personal data in a way that maximizes security and confidentiality.

Cookies and Other Tracking Information

"Cookies" are small pieces of information that are stored by your browser on your computer's hard drive. Cookies may be used on some areas of our website. A cookie is generated by a software application on a sites server which enables it to customize services to the interests of the user by tracking the user's navigation through a small text file on the user's hard drive.

It is Ascend Medical's policy to use cookies to make the use of our websites easier for visitors and to facilitate efficient registration procedures. In most cases, a visitor can refuse a cookie and still fully navigate our website. Our cookies do not contain information that allows us to identify you personally.

Access and Copies

You have the right to review, inspect or receive a copy of the medical information that we keep about you or anyone else that you have legal authorization to access medical information about. Please note that we may charge you for our costs related to your request. We may deny your request in very limited circumstances.

Accounting of Disclosures: You have the right to receive a list of your medical information disclosures, except for disclosures related to treatment, payment or healthcare operations that do not require your consent. You may submit a written request for a time-period up to six years from the date of disclosure. Your first request in a 12-month period is free. After that, we may charge a fee for additional requests.

Amendments

You have the right to submit a written request to amend your medical information, if you believe that information in your medical record is incorrect or that information is missing. We may deny the request if it is not in writing or if it does not include a reason to support the request.

Opt-Out Options

We may use and disclose your medical information in a health information exchange (HIE) with other providers who treat you, when raising funds or conducting marketing campaigns. If you wish to opt-out of any of these activities, you have the right to request to do so in writing. If after choosing to opt-out you wish to opt-back-in you may also do so in writing.

Restrictions

You have the right to submit a written request to restrict how we use or disclose your medical information. We will send you a written response informing you about our ability to honor your request.

Alternate Communications

You have the right to request that your medical information be shared with you in a confidential manner, such as at work rather than at home. If you request for us to email your medical information to you, we will do so securely unless otherwise authorized by you or your legal designee.

Copies of our Notice of Privacy Practices

You have a right to receive a copy of our current Notice. If this Notice was previously sent to you electronically, you may request a paper copy at any time.

Notification of a Breach

You have a right to be notified in writing if there is a breach in the privacy or security affecting your medical information.
