As an existing patient of Dr. Bernard or Nassery (prior to partnership with Ascend), what will change about my care and what do I need to know?
Why are Dr. Nassery and Dr. Bernard Joining Ascend? Joining Ascend Medical allows us to:
- Add more clinicians, including primary care and behavioral health specialists.
- Upgrade our space to better serve the vibrant and growing Old Fourth Ward community.
- We are still part of the Piedmont Clinic.
- We will continue to collaborate with the specialists we have had a relationship with for years.
Will my Doctor still be in the same location? Yes! Your provider will continue practicing out of the same office at 619 Rankin St.
How will my medical records be transferred? After you have signed your consent to continue care with your provider your medical records will be securely transferred to Ascend Medical to ensure continuity of care.
Will my insurance still be accepted? Ascend accepts most major insurance plans. As part of the Piedmont Clinic, we continue to accept all plans that the Clinic does. If you are an existing patient of Dr. Bernard or Dr. Nassery and previously used insurance accepted by them, your insurance will also be accepted by Ascend!
Will my care plan change? Your care plan will remain the same unless your doctor discusses any necessary changes with you. The goal is to ensure that there is no disruption in your care during this transition.
What if I have an upcoming appointment? We will be transferring appointments to our new system, so if you have an appt. Scheduled, we will be seeing you here at Rankin Street as scheduled.
How will I be notified of transition details? Your appointment will remain unchanged and will be honored. However, if for any reason your appointment needs to be rescheduled, we will be available to assist you
Who do I contact if I have questions during the transition? You can contact our office with any questions or concerns. We are here to support you and ensure a smooth transition.
Will the services offered as part of Ascend be different? At Ascend, we aim to offer the same services you are accustomed to and more!
What steps do I need to take to ensure my care continues smoothly? A signed consent forms for the release of your medical records.

Ascend accepts most major insurance plans. As part of the Piedmont Clinic, we continue to accept all plans that the Clinic does. If you are an existing patient of Dr. Bernard or Dr. Nassery (prior to their partnership with Ascend) and previously used insurance accepted by them, your insurance will also be accepted by Ascend!
Visit our insurance page to browse plans accepted by Ascend. If you have a copy of your insurance card, we would be happy to check on your specific insurance provider for you.

How do I prepare for a Telemedicine visit?
- Technology Setup: Please ensure that you have a reliable internet connection and a device (such as a smartphone, tablet, or computer) with a camera and microphone. This will allow you to interact with our healthcare provider seamlessly. If this is first time enable camera and microphone
- Create a Quiet Space: Find a quiet and well-lit area in your home for the telemedicine visit. Minimizing distractions will help you and our provider communicate effectively. We cannot see you if you are driving or in an unsafe
- Medical Records: If you have any relevant medical records, prescriptions, or a list of medications you are currently taking, please have them handy for reference. If possible, please send your records to care_navigator@ascendmedical.com
- Be Prepared to Share: Be open and honest about your symptoms, concerns, and any changes in your health. This will help our healthcare provider make informed decisions.
- Questions: Feel free to prepare a list of questions or topics you'd like to discuss during the telemedicine visit.
How do I cancel or reschedule my appointment?
To cancel or reschedule your appointment, please feel free to contact our office at 404-994-1364 during our regular business hours (7:00am-7:00pm EST). We understand that unexpected situations can arise, and we will do our best to work with you to find a more suitable time.
Please note, cancellations made with less than 24 business hours notice are subject to a cancellation fee. Our fees are $49 for Family Medicine and $99 for Behavioral Health appointments.
